Registration Details

   Registering of Players

All Players must have their own individual passport which is supplied to them from Central Warwickshire.

All Managers please note that in the back of the passports their is

Important Medical details which under Central Warwickshire Rules it is Compulsory they have to be completed !!!

To register your players you must complete the following:

Complete the players passport

Complete a players registration form which must be signed by the Parent or Guardian with all details supplied as stated.

Provide 2 Matching Passport sized photos glued into the required position one in the passport and one on the registration form.

One original form of identification out ot the following

Original passport, Original Birth Certificate or Medical Card

Players Passports must be taken to all league games for opposistion manager to inspect this is a Central Warwickshire Rule
